Emergency, Liability, Contact, Photo Release and Signature By initialing the following you indicate you have read and understand and will abide by each of the statements: * In case of emergency, I understand every reasonable effort will be made to contact me. In the event I cannot be reached, I hereby give my permission to the nurse or physician selected by the Beaver Creek Youth Camp staff to hospitalize and secure medical treatment (including lab tests, x-rays, surgery, etc.) for my child, and accept full financial responsibility for said treatment(s). I also give my permission for the above listed participant to participate in all scheduled activities both on and off camp property. (If there are any activities you do not permit your child to participate in, they must be listed on the camper's medical form or provided in writing to the camp manager before camp begins. * I understand that my child will be doing some or all the following activities and that there are additional physical risks associated with each of these activities. My child and I agree that my child will participate in camp activities within the limits noted on the health forms by me or physician. I acknowledge and assume all the risks for my child associated with all camping activities as well as the additional physical risks for my child associated with each of the following activities: Hiking, Swimming, Low Adventure Course, Climbing Wall, Mountain Biking, Cannoning or other boating type activities, Field Sports, and Horseback Riding (which requires a separate Liability Release form). * I release Beaver Creek Camp Commission, its directors, employees, and volunteers from any and all actions, causes of actions, liabilities, claims, demands, costs and attorney's fees incurred by me by reason of damage, loss, injury, or suffering which may occur on or in connection with Beaver Creek Camp activities. Further, the undersigned agrees to indemnify and save harmless Beaver Creek Camp Commission, its directors, employees, and volunteers against any and all liabilities, loss, damages, costs and expenses which the said Beaver Creek Camp Commission, its directors, employees, and volunteers may hereafter suffer or incur by reason of any claim for personal injury or property damage or claims of any kind or nature whatsoever made by any person including the camper identified above, arising out of injuries or damages or both sustained by the camper identified above which may occur on or in connection with activities of Beaver Creek Youth Camp. * My child and I understand that campers may not have/use cell phones or internet accessible devices while at camp. My child and I agree that my child will act in a responsible way, respecting other people, as well as camp property and facilities. My child and I understand that my child's participation in camp may be terminated if my child acts in a negative or threating manner towards other people or the camp property. * I have read and understand the most current Beaver Creek Parent & Guardian Handbook. * I give permission to Beaver Creek Camp Commission and their designated associates and agents to use any photographs, video footage, and audio recordings taken of my child in any way they deem reasonably appropriate, including websites used by Beaver Creek Youth Camp.
